Performance Improvement Plan: Forms

A performance improvement plan (PIP) is separate from an annual performance review. A PIP represents a formal agreement between the supervisor and employee that outlines specific employee goals or performance standards that need to be met and what the employee must do to meet them more effectively. A good PIP should include:

  1. The performance deficiencies.

  2. Measurable improvement expectations.

  3. A reasonable, appropriate time frame for the employee to improve his or her performance.

  4. Detailed consequences of continued underperformance.
