General Information

As an urgent care facilty we strive to provide accessible and patient centered wellness services.

Medical records are confidential and are protected by law. Therefore, information in a student's medical records, absent an order of the court (subpoena or warrant), may not be disclosed, discussed, or copied without the student's written consent, except in the case of public health matters where certain diseases are required to be reported to Contra Costa County Health Services. If medical information is requested by either a private Clinician or by the Health & Wellness Center, this must be in writing with a release of information form signed by the student.

All full-time undergraduate students are eligible to use the Health & Wellness Center, no matter what insurance a student may have.

Saint Mary's College faculty, staff, and graduate students* are not eligible to use the Health Center at this time, except in the case of emergencies.

The Health and Wellness Center (H&WC) does offer flu shots, at a charge, to all faculty and staff.

* Exception: International Graduate students (who must take the SMC-SHIP can be seen at the H&WC)

Medical Excuses

The Health & Wellness Center does not issue class excuses but can provide documentation that a student was seen at the H&WC if needed.

Medical Requirements

Prior to starting classes at Saint Mary's, all undergraduate students must complete the following:

  • Medical History Form
  • Immunization requirements: immunization records uploaded, showing that all required immunizations have been met.
  • Tuberculosis (TB) Screening/Testing Form: with an upload of TB test results, if indicated.

All of these requirements must be completed electronically at the Health and Wellness Portal by August 15, 2020 for students starting in Fall 2020 to prevent a registration hold.

Everfi Courses

Information for accessing the two required Everfi Courses- AlcoholEdu for College and Sexual Assault Prevention for Undergraduates- will be sent to every incoming Fall 2020 undergraduate student through their Saint Mary's College email around June 15, 2020. Both courses must be completed by August 15, 2020 to prevent a class registration hold.
